Case of Sore Throat, Joint Pain, and Hypotension

A previously healthy 19-year-old female college freshman was seen for a sore throat and fatigue. A Monospot was positive, and a rapid screening test for group A streptococcus was negative. Over the course of the next several days, she developed diffuse myalgias, pain in her right elbow and left ankle, and profound hypotension that was only partially relieved by fluids and pressors. What caused her symptoms?
