A 35 years old male, presented to u with sudden onset of shortness of breath and pin point petechiae all over his body, he has a preceding history of leg trauma that resulted in frature of femur 1 week ago......

what is the probable dignosis? and what test would u perform to support ur dignosis?

(this case was mentioned by dr.Qassim sultan in one of my sessions!!!)

Dx-------Fat embolism
Ix-------Chest X-ray,Blood test(PT,PTT,platelets...)

we have to do spiral ct and ventilation-perfusion ratio

hi all, the diagnosis is pulmonary embolism from the history and the presentation...

there are types of emboli causing pulmoray embolism, the commonest of which are from DVT, but other types as FAT EMBOLI that may originate from fracture may also cause pulmonary embolism...

in addition to pulmonary embolism, fat emboli may cause disseminated intravascular coagulation (DIC), which may present with small petechai due to decrease in platelet count...

so this patient has presented with PE & DIC...
to confirm the diagnosis of fat embolism, general urine examination is done for fat droplets...
