a 60 year old man faints each time he shaves and sometimes when he's wearing a tie, he faints too, what is the diagnosis?

As far as im oriented I think you are talking about a case of :

Carotid Sinus Hypersensitivity.

yeah guys u r right, it's carotid sinus hypersensitivity. to confirm we do carotid massage (but not in patients with carotid diease) while patient on ECG and we take blood pressure too. if the sinus pauses for 3 seconds and systolic blood pressure drops 50 mm Hg or more then carotid sinus hypersensitivity is confirmed. dual chambre pacing helps
