Thanks alote docters for ur reply.
The important sign I want to mention is the air bronchogram in the area of consilidation & I put this film to see the differnce from the previous film of pulmonary infarction in the site of the right middle lobe,
thanks hope to learn from ur topics more.

I see an opacity in the right upper lobe which seems well defined at the lower border of the lobe so I think it is Collapse or Atelectasis of right upper lobe

Well ...

I'm not quite sure but I can see that the Rt. Dome of diaphragm is elevated and this could be due to hepatomegaly ... I'm not sure sorry.

Well, Nice film Dr. Zaid ..

If U concentrate on the Diaphragm, there is Lucent area between the Diaphragm & the Abdominal Organs ( black arrows ), which means " Air Under Diaphragm " ..

Which could be :
1- Ruptured Viscous ..
2- Perforated DU ..
3- Pneumoperitonium ..
& others ..

As U see, there is a :
- Non-Homogenous Opacity of the Upper Right Lobe ..
- The Transverse Fissure is Pulled Upward ( white arrows ) ..
- If U concentrate on the Right Hilum, it is Elevated Upward more than its normal position which should be Lower than the the Left Hilum ( big white arrows ) ..
& these Pulling effects are mostly due to Collapse or Atelectasis of the Right Upper Lobe ..


I hope this is sufficient to enlight U about that CXR ..

I have one question can u tell me please how to differentiate between shadowing of consolidation and collapse and i will thankful bcmf/ill.gif

I think that both cases have some similarity in x-ray shadowing but the main thing to differentaite between them is the shifting of mediasinum or lung fissures...
I mean in cases of Atelectasis you can uasually notice shifting of mediasinal contents (hilum of lung, trachea...etc) or lung fissure toward the collapesd lobe or segment...

While in Consolidation usually there is no such shifting or at least not significant shifting of lung fissure...
