I think it is either :
1- Coarctation Of the Aorta ( COA )
Because there is Rib Notching (white arrows), & there is Bulging of the left ventricle (black arrows) due to left ventricular hypertrophy ..

2- Pott's Disaese ( Tuberculosis of the Spine )
Because there is Scoilosis of the spine (black straight lines ) & this may be associated with TB Pericarditis & this explain the Enlarged Heart ..

I have no clue for dx but i can describe bcmf/blush.gif what i see

the diaphragm is a bit elevated and over lapping with the heart shadow on the left (red arrow)
still the right dome is more elevated then the left(normal) but still i think it is elevated could it be paralysis of the diaphragm ?!!!

the second thing is about the skeleton look at the spinous process they are not centrally located (green arrows) this goes with scoliosis.

good job dr.Ahmed the correct answer is Coarctation Of the Aorta ( COA ),
cause of prominent rib notching and this case presented with hypertesion& radiofemral delay, and for the spine there is nothing wrong with it but the patient a little bit rottated during taking the film.

But, if U concentrate well on the spine, there is a Scoilosis, & it is not related to the patient position ..

Well dr.Ahmed I agree with u that there is some degree of scoilosis but in xray report the radiologist didnt mention that & say that there is some patient movement,
anyway the important thing is the COA diagnosis and I hope all get benifit from the film. rolleyes.gif
