they confirm their dx in hospital only with laprotomy..

Well... if they needed Laparotomy to finally diagnose her condition , I guess we need to do that too !!! biggrin.gif
but if it wasn't mesenteric infarction, i guess It's something related to Warfarine abuse ..!! bcmf/confused1.gif not sure really!!

dr samar is so near from the Dx

it is warffarin over dose... which cause intrabdominal bleeding ... then this bleeding develop 2 infection (peritonitis) causing the picture of acute abdomin

by laprotomy they found clots of blood in the abdominal cavity..

thanx for the answer.. but she didn't have history of malaena? only intraperitoneal bleeding!!!

this is very nice case i saw it in the hospital

16 years old boy ..had history of recurrent attacks of jaundice..
his chief complaint was joint pain and abnormal movement

when i examine him i found..hepatomegally ...jaundice.. hyper tonia with coarse tremor..

what is ur Dx?

A 44 year old patient who is a known case of parathyroid adenoma presented with polyuria and polydypsia. Examination is normal, serum calcium level is elevated.

How can you explain his presentation?


p.s.: we were asked this question during the oral exam of the Medicine course [heads up 6th graders]

well as an answer parathyroid adenoma will secrete parathyroid hormone which will cause hyercalcemia and one of the complains of hypercalcemia is polyurea and polydepsia
